Make a real difference for people living with kidney disease across Wales

At Kidney Wales, we’re here to support, empower and advocate for people affected by kidney disease. We’re now looking for a Patient Information and Support Manager, someone compassionate, organised and proactive, to lead our patient services and help us make a meaningful difference every day.

This is a unique opportunity to shape the way we deliver information and support to patients and families across Wales affected by chronic kidney disease. Working closely with healthcare professionals, partner organisations and colleagues, you’ll ensure that every person who reaches out to Kidney Wales can access clear, timely and trusted support

About the role

As Patient Information and Support Manager, you’ll:

  • Lead the delivery of our patient information and support services across Wales.
  • Provide person-centred support to kidney patients, their carers, and families across Wales, responding promptly and compassionately to enquiries. This includes offering practical guidance, low level emotional and well-being support, and signposting or referring to appropriate services, ensuring patients and families can achieve their desired outcomes.
  • Work with healthcare professionals, renal units and third-sector partners to identify needs and design impactful projects.
  • Create patient-focused resources for our website, social media and e-newsletters.
  • Work with the team to lead/support the design, coordination and delivery of events, webinars and conferences for the kidney community.
  • Manage and maintain accurate patient records within our CRM system.

About you

We’re looking for someone who can bring empathy, professionalism and energy to this vital role.

You’ll ideally have:

  • Experience delivering individual, person-centred support to people living with a long-term chronic health condition.
  • Strong interpersonal skills – able to listen, support, and communicate sensitively.
  • Excellent written and digital communication skills, with the ability to produce accessible and engaging information.
  • Confidence working independently and managing multiple priorities.
  • A good understanding of safeguarding and professional boundaries.
  • Willingness and ability to travel regularly across Wales to visit renal units and attend meetings in the community.
  • Demonstrable experience of working effectively within a team, contributing to shared goals through collaboration, active participation in team meetings, peer support sessions, reflective practice, and service development activities.

 

Desirable ability to speak Welsh
