About the role
Dementia Links is a collaborative service provided by Cardiff and the Vale University Health Board and Alzheimer's Society Cymru in South East Wales. Its focus is to improve the care pathways between primary and secondary care services for people affected by dementia.
This is an exciting opportunity to make a real difference to the lives of people living with Dementia in North/West Cardiff. We are looking for an experienced and passionate individual to join our existing team to deliver high quality services within this geographical area.
Working closely with the Memory Team and primary care specialists, you will become an integral part of their unit providing a multi-faceted support service to those affected by dementia.
Your support will assist people affected by dementia in maintaining their independence.
Your guidance will help to improve their sense of well-being and put them in more control of their lives. The service is unique to each service user as it is based upon their personal circumstances and support needs.
You will offer support and guidance to those newly diagnosed with dementia.
You will also assist by providing information and knowledge as well as aiding service users to access appropriate services.
Support will be given in a variety of ways, whether in the client's own home, at service offices or at other locations, face to face, by phone, letter or email.
The use of a vehicle and the ability to travel independently is therefore essential to this post.
We are looking for a committed, dynamic individual with excellent communication and organisational skills.
We are seeking an individual that is passionate about making a positive difference to the lives of those affected by dementia.
To be successful in this role you will need to evidence strong organisational and time management skills in order to manage your caseload of clients effectively.
You will be required to navigate both Alzheimer’s Society Cymru and NHS systems therefore you will need strong IT skills and have a proven record of accuracy and attention to detail.
You should possess NVQ level 3 (or equivalent) or have an understanding of dementia and the needs of those living with dementia and their carers.
Your ability to assess client needs along with the understanding of the need for client confidentially is essential to providing a valuable and worthwhile service.
